The hole doesn't go through to the inside. It's millimeters deep, but good news is that the vinyl is molded in the color of his mouth, so yeah, there's a hole, but its the same color.
The hot/warm water will soften the vinyl and loosen the glue. You can use the same method to pry heads from neck plugs. Just remove Carnage's head, and bathe it in the water, although, I suggest holding it and only submerging the mouth that way you dont soften the whole piece, just what you need to work on. So yeah, put it in some water and every so often tug on/ move the tongue around to loosen the hold. Eventually, it'll come out. It might take some force (be careful!) and some heat (again, be careful!) but it will come out. Be careful not to get the vinyl
too hot, then you might risk deforming the head while you hold/pull on it. I suggest tossing some water in a pot and get it to just below boiling. Boiling will be too hot, and drinking temp a bit too low. You
can use lower temperatures, but it'll take longer. Good luck, and be gentle!
