I purchased 4 foam boards 30"x40" from my local Staples to create the shelves. I posted the template dimensions and a picture of the finished piece for ya.
Also, I do recommend reading Alex's tutorial. He explains the process more thoroughly. And he's got it set up nearly perfect for a four tier setup. The only problem I came across was making the total width of the shelves at 15". I couldn't slide it in the display case. I adjusted the total width at 14-1/8" and it fit like a glove.
